You understand our Puppy Package is training sessions for your puppy to be acclimated for grooming.

The package includes 3 bath and tidy sessions, booked 2-3 weeks apart. (Must be completed in a 10 week time frame. If not completed in the time frame you understand we can deny performing the remaining package.)

*The package includes our 2-Step Bathing process, conditioner, introduction to blow dryer, brush out, ear cleaning, nail trim, introduction to Dremel; Haircut dogs also include a Face, Feet and Sanitary Trim.*

Some services may not be performed during a session if the puppy becomes too stressed. We put the animal first and want to create a positive experience while they’re in our care.

*Haircut:

At the third session, if the puppy is ready we can perform a full body haircut. We will only do what is comfortable for them, and will not push through any service. There will be an additional charge as we credit the remaining amount from the package toward a full groom price.

Prices vary due to breed, size, coat condition, and behavior.

I, the undersigned, hereby acknowledge that my pet, as listed above, has significant matting and/or tangles in its fur. I understand that this matting can cause discomfort, pain, and even injury to my pet during the grooming process.

I understand that the grooming salon cannot be held responsible for any injuries or adverse reactions caused by the matting, and I release them from any liability. I also understand that my pet may require additional services, including but not limited to dematting, which may result in additional charges.

I authorize the grooming salon to take any necessary actions to ensure the safety and well-being of my pet during the grooming process. I understand that if my pet's matting is too severe, the grooming salon may be unable to safely remove the mats and may need to shave the affected area or the entire coat.

I understand that there may be an additional fee for the time taken to de-mat my pet safely, and that fee is based on time taken. ($1 per minute of Dematting)

I have read and understood the contents of this matted release form and agree to its terms and conditions. I certify that I am the legal owner or agent of the owner of the pet named above and have the authority to execute this release.
